Assassin's Creed Black Flag Best Graphics Mod

Hi
I'm playing Assassin's Creed IV on my PC but there's a lot of lag. The game frames freeze a little every second.
I've looked for answers with google over the internet and tried vsync with nvidia but this is not a satisfying solution.
I don't understand all the graphic options in the game settings and don't know what to activate or not.
What would you recommend ?

I've tried to optimize Nvidia via the control panel but it reads that this game can't be optimized.
